Chronic Care Plans

Living with a long-term medical condition can involve regular appointments, medicines, monitoring and care from different health professionals. A Chronic Care Plan helps bring these together into a clear, coordinated approach to your health.

At DC Medical Centre, your GP can work with you to identify health goals, plan treatment, organise appropriate referrals and review your progress over time.

Bulk billed care plan consultations

DC Medical Centre bulk bills eligible Medicare Chronic Care Plan preparation and review consultations. There is no out-of-pocket fee for these eligible care plan services at our practice.

Understanding your plan

What is a GP Chronic Condition Management Plan (GPCCMP)?

A GPCCMP is a personalised written plan prepared with your GP to help manage one or more chronic medical conditions. It records your health needs, treatment goals, agreed actions, care providers and arrangements for follow-up.

From 1 July 2025, GPCCMPs replaced the former GP Management Plans (GPMPs) and Team Care Arrangements (TCAs) for new plans.

Who can access a plan?

GPCCMP Eligibility

To be eligible for a Medicare-funded GPCCMP, the following requirements generally apply:

01

A chronic condition

You have at least one medical condition that has lasted, or is likely to last, six months or more, or a terminal medical condition.

02

A plan would help

Your GP considers that structured care planning would benefit your health. A diagnosis alone does not automatically guarantee a plan.

03

Your usual GP or practice

The plan is generally provided through the practice delivering most of your ongoing GP care. MyMedicare-registered patients must use their registered practice.

People living in residential aged care generally access different multidisciplinary care planning arrangements rather than a GPCCMP. Medicare eligibility and previous care plan history are checked before claiming.

Examples

Conditions that may benefit

There is no fixed list of qualifying diagnoses. Your GP considers how a structured care plan could support your individual needs.

Practical support

What do you get with a GPCCMP?

A personalised care roadmap

  • A documented summary of your chronic health needs and priorities
  • Agreed treatment goals and practical steps to work towards them
  • Coordination between your GP and other involved health professionals
  • Medication, monitoring and follow-up arrangements as appropriate
  • A copy of your agreed plan and regular opportunities to review it

Access to Medicare-supported services

Up to 5

Individual allied health services per calendar year for eligible patients, shared across providers and subject to GP referral and Medicare rules.

Depending on your condition, your GP might refer you to a:

  • Physiotherapist or exercise physiologist
  • Dietitian or diabetes educator
  • Podiatrist or occupational therapist
  • Another eligible allied health professional

The five visits are not five per profession. Additional services may be available under specific Medicare rules, including for Aboriginal and Torres Strait Islander patients and eligible people with type 2 diabetes. Allied health providers may charge a gap fee even though DC Medical Centre bulk bills its eligible care plan consultations.

How it works

Your Care Plan Appointment

1

Meet with your GP

Discuss your medical history, current treatment, ongoing difficulties and what you would like to improve.

2

Develop an agreed plan

Your GP outlines suitable health goals, treatment actions, services and who will be involved in your care.

3

Arrange referrals if needed

Where clinically appropriate, your GP may provide referrals to eligible allied health professionals.

4

Review your progress

Return periodically to assess progress, update treatment goals and adjust the plan as your needs change.

Plan and review timing

Every 12 months

A new GPCCMP can ordinarily be prepared once every 12 months.

Every 3 months

A GPCCMP review can ordinarily be claimed once every three months.

Earlier preparation or review may be possible in exceptional circumstances, such as a significant change in your condition, if Medicare requirements are satisfied.

Important things to know

Your GP decides what is appropriate

A care plan does not automatically entitle you to a set number of referrals. Your GP recommends services that are clinically relevant to your needs.

Check allied health fees

Medicare generally provides a rebate towards eligible allied health appointments; it does not guarantee those appointments will be free.

Existing older plans

Plans created before 1 July 2025 may continue to support access to eligible services under transitional arrangements until 30 June 2027.

Bring useful information

Please bring your current medication list, relevant specialist letters, investigation results and the names of healthcare providers involved in your care.
