RECOMMENDED APPOINTMENT
Book an appointment for clinical assessment. Medical certificates are issued only when supported by your GP's assessment, and an in-person examination may be required.
Our GPs assess requests for certificates supporting absence from work, university, school and other responsibilities due to illness or injury. A certificate is a clinical document; the practitioner must be satisfied that the information is accurate and medically justified.
Depending on your circumstances, a Standard in-person consultation or Video Consultation may be suitable. Your GP may request further assessment before deciding whether a certificate can be issued.
The issue date must be the actual date your practitioner prepares the certificate. It cannot be altered to make it appear that you were seen or the certificate was written earlier. In limited circumstances, a GP may determine that an earlier period of incapacity can be described based on the available clinical evidence, but this is entirely at the practitioner's discretion and is not guaranteed. Please seek medical assessment promptly when you become unwell.
Your GP can assess illness or injury affecting your ability to work and, where appropriate, certify a period of incapacity. Employer and workplace requirements may vary.
Some institutions require specific medical certificates or special-consideration forms. Check the required format and deadlines before attending.
Medical evidence for caring responsibilities may be available where a qualifying illness, injury or unexpected emergency is involved and sufficient clinical information is available.
If you need a medical certificate for university and your institution requires a specific template, download the medical certificate or special-consideration form from your university's website.
You must either print the form and bring it to your appointment or email it before attending to reception@dcmedical.com.au. Please check any mandatory fields and submission deadlines. Your GP can complete only information supported by their clinical assessment; completion or university acceptance is not guaranteed.
Select Standard Consultation (15 minutes) or Video Consultation using our booking link.
Discuss your symptoms, functional limitations and relevant dates. Your GP may perform an examination or require additional information.
If clinically appropriate, your GP prepares a certificate dated on the day it is issued, using your institution's form where required and feasible.
Video appointments may be appropriate for some certificate requests, subject to the GP's clinical judgement. Not every condition can be assessed adequately by video. The doctor may recommend an in-person visit or decline to issue a certificate if there is insufficient evidence.
Any Medicare telehealth rebate depends on current eligibility and billing rules. Video availability does not guarantee Medicare coverage.
Standard consultation fees apply. Charges and any applicable Medicare rebates depend on the appointment and individual eligibility.
See our Fees page for current consultation charges.
A medical certificate normally confirms the clinically appropriate period of incapacity without unnecessary disclosure of your diagnosis. Employers and educational institutions may set their own evidence rules or request specific information. Confirm their requirements before booking, as DC Medical Centre cannot guarantee that a third party will accept a certificate.
For general workplace evidence requirements, see Fair Work Ombudsman guidance →.
Book a Standard Consultation (15 minutes), or a Video Consultation if clinically appropriate. An in-person examination may still be required.
A video consultation may be suitable if your GP can make an adequate clinical assessment. A certificate is not guaranteed, and the GP may require an in-person review.
No. We cannot backdate the date a certificate is issued. A GP may, at their clinical discretion, describe an earlier period of illness if there is adequate evidence; this is not guaranteed.
If your university requires its own form, download it from your university website and print it to bring to your appointment, or email it to reception@dcmedical.com.au in advance.
Not necessarily. Universities may require a specific template, supporting details or submission deadlines. Check your institution's instructions before your appointment.
Your GP can assess your health and decide whether a certificate confirming incapacity for work is appropriate. An appointment does not guarantee a certificate.
Your GP may assess whether supporting medical evidence for carer's leave is appropriate. Additional information or an examination of the person requiring care may be necessary.
Medical certificates generally need not disclose a detailed diagnosis without your consent. Your GP will provide clinically appropriate information while respecting your privacy.
Your GP determines the clinically supported period of incapacity. The length of a certificate cannot be guaranteed before an assessment.
Yes. Depending on your employment arrangements, your employer can request reasonable evidence for a single day or even part of a day of sick leave.
Standard consultation fees apply. See our Fees page for current rates; Medicare eligibility and the consultation type may affect rebates.
They may contact the practice to verify a document's authenticity, but medical information remains confidential and will not be shared without an appropriate basis or consent.
Book a Standard Consultation (15 minutes) or Video Consultation. University students should bring their required university template or email it to reception@dcmedical.com.au before the appointment. Certificates cannot be backdated.
