REQUEST YOUR RECORDS
If you previously attended another GP practice, we can request your medical records on your behalf.
There is no fee from DC Medical Centre to request your records. Simply complete our online transfer form and our team will contact your previous practice.
Access to your clinical history helps your GP understand previous diagnoses, medicines, allergies, immunisations, investigations and treatment plans. This can support continuity of care and avoid unnecessary duplication of tests.
You do not need to have your entire previous record before your first appointment, but please bring any important information you already have, especially about current medicines, serious allergies or ongoing treatment.
Our records request process is free to patients.
We contact your previous practice after receiving your completed form and relevant authority.
The previous practice controls how and when it releases records and may apply its own charges or requirements.
Use our Medical Records Transfer Form. Include your details and the name of your previous practice as requested.
Once your form is submitted, DC Medical Centre requests your records using the details and consent provided.
Your former practice responds and provides available records or a relevant clinical summary by an appropriate secure method.
Your previous practice generally needs appropriate patient consent or authority before transferring health information to a new provider. Completing the transfer form helps our team make the request. For children and people represented by guardians, the previous practice may need to confirm the requester's authority. Read NSW privacy guidance →
The timeframe depends on your previous provider, the completeness of the request and any further consent or identification checks. We cannot promise that records will arrive before a specific appointment. If you have been waiting and need an update, contact reception.
Depending on the former practice's systems and your clinical needs, you may receive a transfer summary or more extensive records containing medication history, allergies, investigations, immunisations and relevant correspondence. We can follow up for missing clinical information when necessary.
No. DC Medical Centre does not charge you to complete our records request process. Your previous practice may have its own policies or charges for supplying records.
Complete our medical records transfer form using the link on this page. We then send a request to your previous practice on your behalf.
Normally, we will request your records once you submit the form. The previous provider might contact you to confirm consent or request further details.
Timing depends on how quickly your previous practice responds, whether consent and identification are complete, and the size of the file. We cannot guarantee a fixed timeframe.
The previous practice may provide an appropriate clinical summary or relevant records for continuity of care. If more information is needed, we can follow up with the provider.
Transfer information can include relevant medical history, current medicines, allergies, immunisations, recent test results and important clinical correspondence, depending on what your previous practice provides.
Where suitable secure clinical messaging or another secure transfer method is available, practices can share records electronically. Your previous practice determines its available methods.
Complete the form with the practice's former name and details, if known. We may need to identify the current custodian of the records before a request can progress.
A parent, guardian or authorised representative may be able to make a request, but authority and the patient's privacy rights must be verified. Additional consent may be required.
Yes. Please bring any medication list, recent results or important medical information you already have so your clinician can provide safe care while we wait.
Contact reception so we can check whether your request was received and, where appropriate, follow up with the previous practice.
No. My Health Record may hold useful shared documents, but it is not necessarily a complete copy of your previous GP's clinical record. A separate transfer may still be helpful.
Complete the online form and DC Medical Centre will request your records from your previous practice for you. There is no DC Medical Centre fee for making the request.
